Nurse Practitioner in Chicago, IL
IAMACF Seva Community Health Clinic is looking for a Nurse Practitioner to work three days a week (preferably Tuesday, Wednesday, and Thursday) based in West Ridge of Chicago, IL. The physical address of the Seva Community Health Clinic is 2546 W Peterson Ave, Chicago, IL 60659

The Free Health Clinic is a volunteer physician-based, free charitable clinic operating in the West Ridge neighborhood of Chicago serving South Asian and many diverse underserved immigrants and refugee populations who live in the area. Indian American Medical Association Charitable Foundation (IAMACF) Community Health Clinic, a nonprofit, was established on October 7, 1994, to provide free health care for underserved communities and people without health insurance. Initially, it was housed at Indo-American Center. In 2003, it was moved to 2645 W Peterson Street when a generous donation from Dr. Nimmagadda in his son’s name purchased the building and converted it into a Clinic facility.
After its complete renovation, Seva Community Health Clinic started. IAMACF Seva Community Health Clinic has six private examining rooms, a large waiting area, a conference room, a pharmacy, and a lab testing area to service appointments and walk-in patients. The mission of IAMACF Free Health Clinic is to provide basic primary care, preventive medical care, and public health education with dignity and respect for all. In 1998, IAMACF obtained 501©(3) nonprofit status and began providing free weekend medical services.
Duties:
- Patient exam/consultation (HPI, medical history, physical exam, review of pertinent medical records/imaging) Typical patient volume 10-20 patients per day
- Documentation, detailed and timely (SOAP note- description of injury, HPI, PMH, and causation in addition to physical examination)
- Diagnosis
- Formulating written assessments and treatment plans
- Provide therapies and treatment for a wide range of pain conditions
- Order/review diagnostic imaging/records/labs
- Communicate and consult with medical staff regarding patient needs
- Discuss actions with patients that will help improve their lives/ Patient Education
- Keep precise medical records for each patient
- Prescribe necessary medications or therapy
- Provide patients with a plan of care details and patient education regarding care plans
Requirements:
- Graduation from an accredited NP program
- Current and valid IL State NP license
- Must have IL controlled substance license
- Must have a current DEA number
- Experience in primary care
